Participation Forms

• Entry Forms and Field Rental Request Forms need to be turned into the Recreational Sports Office on the third floor of the Student Success Center by the registration deadline for the particular activity.
• Field Rental Request Form
• Field Rental Requests are to be filled out by groups/organizations that are interested in using the GSW Intramural Field.
• Individual/Dual Entry Form
• Individual/Dual Entry forms are used in activities that only involve one or two participants.
• Team Entry Form
• Team Entry forms must be filled out by all teams that participate in intramural team sports.
