Immunization records (Those submitted prior to Spring 2020) those submitted after must request records through Medical records must be requested in writing to the Health Center by submitting the form by clicking consent for authorization below: 

Consent for Authorization

The request must include the student's full name, GSW ID#, student's phone number, a complete address, fax number, or email address where the records are to be sent, signature, and appropriate fee. The consent forms can be completed at the Health Center or by accessing the above link. Records may be requested by submitting the consent form in person, or via email ( In order to avoid any delays, please be sure to include all the names you may have used while attending GSW. The patient's information is held in confidence and will not be released without the patient's signed consent. Records will only be released to the patient or a physician.  A student's Health Services records are not part of the University's records. 

  • Immunization (Prior to Spring 2020) and/or Medical records are $1.00 per page. Health Services accepts payment in the form of cash, check, or money order. Credit or Debit card transactions can be processed through the cashier in the business office (229-931-2013).
  • The request will not be processed until we receive payment. Please include a form of payment with your requests to avoid any delays. If you are picking up your records from Health Services, you may indicate on your requests that you will pay at the time of pick up.
  • You may have someone else pick up your request if you send written permission for that person to pick up your request. The person picking up your immunization records only must have proper identification.
